This program provides quality Care Management services to adults in our community, rooted in trauma-informed and person-centered. We support adults develop goals that may include increased independence, getting connected to mental health and physical health care, safety, etc. We collaborate with other providers involved, and ensure they are linked to the community resources they need to stay healthy.
