This program provides quality Care Management services to youth and families in our community, rooted in trauma-informed, person-centered, family-driven care. We support youth develop goals that may include increased coping skills, social skills, communication skills, independence, safety, academic success, etc. We support youth/families in getting connected to the community resources that meet their needs. We aim to ensure all providers involved in the youth’s care are working in the same direction.
